Job Description:

Purpose:Directs all operations within assigned territory or facility, either directly or through subordinates, based on patient-driven census modeling. Directly responsible for reputation, financials, including revenue growth, compliance, strategic planning, employee-engagement and development and partnering with organization initiatives.Responsibilities:CareParticipates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met.Consults with the Executive Director of Hospice to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of personnel.Participates in establishing the functions and qualifications for each service position; Coordinates interdisciplinary team services.Assures ongoing assessment of patient/family needs and supervises development, implementation and evaluation of interdisciplinary group plan of care. Assures physician approval of plans for continuity of medical care.Organizes hospice to delineate and delegate authority, functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.Oversees confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.Investigates and reports any problem relating to patient care or conditions which might harm the patient and/or employee well-being. Addresses and corrects all patient concerns.Immediately reports any accident, incident, lost articles or unusual occurrence to the Executive Director of Hospice.Oversees hospice's ongoing Quality Assessment/Performance Improvement (QAPI) program.In collaboration with peers and Administrator On-Call, provides 24 hours/day, 7 days/week on-call coverage.Conducts patient case conferences, inservices, staff meetings and maintains documentation; Participates in community education projects.Teaches, supervises and counsels the hospice patient and family members about providing care for the patient.Maintains skills and knowledge.StaffProvides supervision of clinical, support and clerical employees, and oversees staffing and assignments.Assists in education/training programs. Participates in planning for the orientation of new employees. Participates in orientation of new personnel as appropriate to team responsibilities.Displays a willingness to support policies and procedures and uses appropriate channels for change of such policies.Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance.Serves as a resource person to employees. Plans and arranges for consultation needs of staff. Provides individual or group support concerning job related stress or issues.Develops a cooperative relationship and communicates effectively and professionally with the physicians.Conducts appropriate supervisory visits which allow for clinical observation of performance of assigned team members.Provides information for inclusion in written performance evaluations for all staff assigned to team in cooperation with appropriate members of the management team.Identifies needs for staff education and assists in the development of inservice programs for interdisciplinary group.BusinessResponsible for seeing that objectives are implemented.Makes routine, scheduled calls on referral sources for the purpose of promoting access to hospice and referrals to the organization.Participates in coordinating hospice's services with services of other community agencies.Accepts responsibility for regular attendance and punctuality, fulfills job requirements without regard to time involved.Works with Executive Director of Hospice in identifying budgetary requirements and determining appropriate use of allocations.Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.Ensures that all necessary supplies and equipment are available.Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he/she will not be available.Serves as a member of the management team in order to address problems associated with the delivery of services and staff development needs, to put forth suggestions for appropriate additions/changes in policies and procedures and to give peer input to enhance and improve the management function.Supervises the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.Performs specific assignments as directed by the Executive Director of Hospice.Participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.Qualifications:Licensed RN, or bachelor's degree in a recognized discipline of the IDG, i.e., RN, SW, SCC, BC, or master's degree in nursing, social work, counseling, health care administration, public administration, businessAt least 1 year in a leadership positionKnowledge of professional principles and practices for patient care disciplines dealing with end-of-life careKnowledge of Medicare and Medicaid conditions of participation and standards for hospice and other health care entitiesDemonstrated knowledge of current leadership and supervisory principles and practices with strong human relation skills; Experience working in a team settingDemonstrated knowledge of continuous quality improvement environments and techniquesAbility to develop and maintain business relationships including good public contact and group presentation skillsDemonstrated ability to handle multiple priorities in an organized manner, make timely decisions and exercise sound independent judgmentAbility to evaluate people and provide an environment that motivates othersDemonstrated effective verbal and written communication skills with individuals and groupsDemonstrated ability to identify problems/issues and confront them; Effectively employ negotiation, conflict management and quality improvement skills to resolve issuesStrong keyboarding and computer skills and the ability to learn work-related softwareCurrent American Heart Association Healthcare Provider certification (within 30 days of hire)Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)Must have valid driver's license, auto liability insurance and reliable transportation if driving is requiredSuccessful completion of pre-employment physical, drug screen, 2-step TB test and background checkWorking Conditions:Physical requirements: Sedentary work exerting up to 10 pounds occasionally, and/or a small amount of force frequently to move objects, including the human bodyPhysical activities: Talking, Hearing, Repetitive Motions, Fingering (picking, pinching, typing, etc.), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)Environment: Work is primarily performed in a climate-controlled environment with minimal safety/health hazard potentialTime: Ability to be flexible with a varying schedule and programmatic requirementsTravel: Required between locationsThese characteristics are representative of those an employee encounters while performing the essential functions of this job.
