JOB SUMMARY / PURPOSE:
Under direction, provides professional nursing care for adult
clinic patients following Iowa Heart Center's (IHC) established
standards and practices. Works collaboratively with physicians,
staff and other health care professionals/ agencies to assist with
care coordination for patients with cardiac diseases. Communicates
treatment plans/proposals to patients and approved support
individuals. Collaborates with other services and disciplines, and
when appropriate, the client's medical physician and/or related
health care professionals/agencies. Ensures safety, best practice
and high quality standards of care are maintained.
ESSENTIAL KEY JOB RESPONSIBILITIES:
1.Acts as a consultant to nursing colleagues and other health care
professionals, providing clinical expertise and recommendations to
medical staff, nurses, and multidisciplinary health care
2.Develops and maintains a collaborative manner of communication
with medical staff, multidisciplinary colleagues, leaders, patients
3.Facilitates the sharing of multiple viewpoints while recognizing,
respecting and valuing differing viewpoints.
4.Maintains and obtains knowledge independently, and bases
decisions on current clinical practice guidelines, trends,
research, data, regulatory standards, legislation and developments
in patient care.
5.Performs concurrent review for key clinical quality/patient
safety indicators and associated documentation; identifying
problems and potential problems with initiation of problem solving
with clinical care-givers.
6.Provides and maintains a system for tracking and trending for key
clinical quality/patient safety indicators and associated
documentation, develops trending/summary reports for presentation
to a variety of audiences.
7.Measures, analyzes, and uses problem solving techniques to
facilitate improvement with key clinical quality/patient safety
measures performance and patient outcomes.
8.Provides quality improvement facilitation and support for
clinical care-givers, with clinical improvement initiatives by
mentoring, coaching and just-in-time education in the methods and
tools of rapid cycle and continuous quality improvement.
9.Demonstrates knowledge and understanding of current clinical
practice guidelines, professional nursing standards, CMS
regulations, regulatory/accreditation requirements, state licensure
laws, national clinical quality benchmarks and organization
policies/procedures; and applies this knowledge and understanding
to the evaluation of patient care delivery; identifying problems,
assisting with problem solving and providing just-in-time, at the
elbow advising, coaching and education to medical staff and
clinical colleagues to facilitate compliance.
10.Identifies clinical education needs and assists with development
of formal and informal education collaborating with Clinical
Resource Nurse, clinical colleagues, medical staff, patients and
families, community, and Network colleagues related to key clinical
quality/patient safety indicators and associated documentation.
11.Actively participates and coaches colleagues in problem
identification and problem solving, using the organization tools
including, but not limited to, 5-Why, Waste Elimination, Safety
12.Actively involved and frequently leads individuals or groups
with performance improvement activities related to key clinical
quality/patient safety indicators and associated documentation
issues with efficient use of human and physical resources.

* Bachelor's degree in Nursing (BSN) required. Master's in nursing
or healthcare field required, and must obtain within five years of
* Licensed Registered Nurse, current in the State of Iowa,
required. Proof of completion of Mandatory Reporter - Child and/or
Dependent Adult Abuse training within three (3) months of hire. BLS
certified by the end of the orientation period (approximately six
