JOB SUMMARY / PURPOSE:

Under direction, provides professional nursing care for adult clinic patients following Iowa Heart Center's (IHC) established standards and practices. Works collaboratively with physicians, staff and other health care professionals/ agencies to assist with care coordination for patients with cardiac diseases. Communicates treatment plans/proposals to patients and approved support individuals. Collaborates with other services and disciplines, and when appropriate, the client's medical physician and/or related health care professionals/agencies. Ensures safety, best practice and high quality standards of care are maintained.

ESSENTIAL KEY JOB RESPONSIBILITIES:

1.Acts as a consultant to nursing colleagues and other health care professionals, providing clinical expertise and recommendations to medical staff, nurses, and multidisciplinary health care providers.

2.Develops and maintains a collaborative manner of communication with medical staff, multidisciplinary colleagues, leaders, patients and families.

3.Facilitates the sharing of multiple viewpoints while recognizing, respecting and valuing differing viewpoints.

4.Maintains and obtains knowledge independently, and bases decisions on current clinical practice guidelines, trends, research, data, regulatory standards, legislation and developments in patient care.

5.Performs concurrent review for key clinical quality/patient safety indicators and associated documentation; identifying problems and potential problems with initiation of problem solving with clinical care-givers.

6.Provides and maintains a system for tracking and trending for key clinical quality/patient safety indicators and associated documentation, develops trending/summary reports for presentation to a variety of audiences.

7.Measures, analyzes, and uses problem solving techniques to facilitate improvement with key clinical quality/patient safety measures performance and patient outcomes.

8.Provides quality improvement facilitation and support for clinical care-givers, with clinical improvement initiatives by mentoring, coaching and just-in-time education in the methods and tools of rapid cycle and continuous quality improvement.

9.Demonstrates knowledge and understanding of current clinical practice guidelines, professional nursing standards, CMS regulations, regulatory/accreditation requirements, state licensure laws, national clinical quality benchmarks and organization policies/procedures; and applies this knowledge and understanding to the evaluation of patient care delivery; identifying problems, assisting with problem solving and providing just-in-time, at the elbow advising, coaching and education to medical staff and clinical colleagues to facilitate compliance.

10.Identifies clinical education needs and assists with development of formal and informal education collaborating with Clinical Resource Nurse, clinical colleagues, medical staff, patients and families, community, and Network colleagues related to key clinical quality/patient safety indicators and associated documentation.

11.Actively participates and coaches colleagues in problem identification and problem solving, using the organization tools including, but not limited to, 5-Why, Waste Elimination, Safety Observations, etc.

12.Actively involved and frequently leads individuals or groups with performance improvement activities related to key clinical quality/patient safety indicators and associated documentation issues with efficient use of human and physical resources.

13.Communicates with internal/external customers/colleagues in a courteous, positive, empathic and professional manner. Always communicates with care and with a calm, clear, and courteous style. Stops and listens when a customer/colleague speaks.

14.Adapts positively to the ever-changing health care environment by supporting technology and process changes. Willing to learn new things. Open to change and maintains a calm, positive manner under stressful conditions.

15.Maintains required department confidentiality and abides by HIPPA regulations.

16.Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.

17.Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development, and participates in medical center and/or departmental education programs.

18.Communicates with and performs duties following the philosophy of Mercy Medical Center and the Sisters of Mercy reflecting the organization's Mission, Values and Guiding Behaviors.

19.Supports and abides by all medical center, departmental and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.

20.Other duties as assigned by management.

Required Education/Certification/Experience:

* Bachelor's degree in Nursing (BSN) required. Master's in nursing or healthcare field required, and must obtain within five years of hire date.


* Licensed Registered Nurse, current in the State of Iowa, required. Proof of completion of Mandatory Reporter - Child and/or Dependent Adult Abuse training within three (3) months of hire. BLS certified by the end of the orientation period (approximately six (6) weeks).


* A minimum of three years of relevant clinical experience in an acute setting required, five years preferred.
* Demonstrates high degree of initiative, self-awareness, judgment, investigational skills, detail oriented and able to set and organize own work priorities and adapt as needed.
* Demonstrates excellent interpersonal communication skills and maintains collaborative relationships with medical staff, clinical colleagues, and Leaders; including public speaking, presentation and writing skills.
* Demonstrates ability to improve department or hospital wide processes through data analysis and principles of research; including data collection, verification, analysis, interpretation and presentation.
* Proven problem identification, problem solving and conflict management skills.
* Strong computer skills including Word, Excel, Power Point, Cerner Power Chart.
* Project management skills with ability to prioritize, manage a team and facilitate groups.
* Ability to work flexible hours and modify workload as needed.
* Experience with data collection, basic statistics and statistical measurement tools, and data analysis.
