Health - Eyerly Ball is seeking a RN in our Des Moines office to
join our Assertive Community Treatment (ACT) team.As a Community
Support Specialist, you will partner with adults who have mental
illnesses assisting them in leading healthy happy lives.$5,000 Sign
on Bonus Available for Qualified Registered Nurse CandidatesAs a RN
you will:
- Provide direct support to prescribers in serving clients
- Collect basic vitals and medication reconciliation during
appointments
- Follow up on provider orders, lab results and medication
prescriptions for clients
- Organize and manage the system to administer medication to
clients as well as integration of medication into the clients
individual treatment plan
- Administer IM injections as per protocol
- Complete all documentation within established guidelines
- Serve as a member of the Assertive Community Treatment Team
(ACT) in the planning and development of treatment, rehabilitation
and support services to clients with severe and persistent mental
illness
- Build and maintain relationship with Medicaid Managed Care
Organizations, Polk County Health Services and other providers in
the service network
- Coordinate nursing care activities in collaboration with other
members of the ACT teamIdeal Candidate:
- Ability to de-escalate situations with calmness and care
- Communicate effective both verbally and in writing
- Passion for improving the well-being of a vulnerable
populationTo be considered youll need:
- Associate Degree in Nursing (ADN) or Bachelor of Science in
Nursing (BSN)
- RN license to practice in the state of Iowa
- Prior experience providing care and intervention to individuals
with mental illnessRequirementsSchedule:
- Monday-Friday, 8am-5pm
- Rotation of late shift, 5pm-7pm
- On call responsibilities to include: carrying the 24-hour
client crisis line phone to respond to respond to inquiries and
potentially deliver medication and/or provide crisis triage
services via phone or on scene. On call hours receive additional
payment of of $20/day during the week and $40/day on Saturday and
