Non-Exempt Reports to: Clinic Supervisor Job Summary/Objective:
Provides primary care nursing, patient education, health care
maintenance, patient counseling, and normative care services to
patients at Peoples Community Health Clinic. Works with providers
and administers nursing care utilizing the nursing process
(assessment, planning, implementation, and evaluation). Protected
Health Information Requirements/Access: • This position will
require the use or disclosure of protected health Information. •
This position will use the Treatment and Health Care Operations
classes of protected health information. • Restrictions on the
protected health information for this position will follow the
Privacy Policies of Peoples Community Health Clinic, Inc. • Use or
disclosure of protected health information not routinely available
to this position will follow procedures assessed by or directed by
management. • Patient Records – Yes Medical Information System –

functions. • While specialized RN positions in pediatric case
management, health coach and Medication Assisted Treatment (MAT)
nurse case manager have functions related to their specific
position all RN positions will be expected to perform these duties.
• Provides primary and normative care for patients. o Prepares
patient for provider visit including explanation of PCMH model and
related services. o Assesses and evaluates health and development
status of the patient, including complete medical and/or social
history, vitals, and measurements according to approved protocol. o
Maintains accurate records and documents appropriate information in
patient’s EHR including notifying patients of test results when
needed. o Reviews patient’s EHR for pertinent information including
obtaining documents from other facilities as needed. o Evaluates
need for referral and makes referrals as needed. o Assists
providers with procedures, including preparation of equipment and
clean-up. o Provides medical therapy or monitoring of that therapy
as directed by the provider or by protocol, including medication,
immunization, and IV fluids administration. o Performs EKGs,
hearing and vision tests, ear lavage, tympanometry, spirometry,
catheterization of the urinary tract and CLIA waved lab testing o
Administers irrigation, including enemas and removal of impactions.
o Collects and labels specimens, including cultures. o Measures and
records physiological and growth indices. • Provides patient
education. o Provides planned educational programs and materials
per evidence-based guidelines related to prevention, health
maintenance and improvement. o Reinforces provider’s instructions
and explains patient’s plan of care. • Provides patient counseling.
o Provides emotional support and an atmosphere of confidentiality
to facilitate discussion of clinic procedures, prevention or
testing for STDs and other communicable diseases. o Provides
support and guidance to clients and families of clients who are
terminally or chronically ill. o Serves as a client advocate to
provide protection of client rights and for complaints regarding
care or service. • Coordinates Provider support. o Inventories
medications, supplies, vaccines, crash carts and exam rooms as
needed. o Functions as coordinator of patient services to
facilitate patient flow. o Participates in Team Huddle/Meeting to
communicate information among providers and care team members. o
Aids the Clinical Representatives in areas of practice which are
beyond their scope and serves as a resource to other staff members.
• Provides symptom assessment to determine level of illness and
patient care needed. o Collaborates with providers to facilitate
patient care. o Provides advice and follow-up to patients regarding
care, including patient education according to protocol. o
Completes medication refills according to protocol. o Schedules
patient appointments as needed. Pediatric Case Management-Provides
case management services for pediatric and adolescent patients
including assessment, planning, referral, coordination, and
follow-up. o Ensures the provision of primary health care services
from birth through adolescence as recommended by AAP. o Facilitates
access of high risk/special needs pediatric patients to state and
local services. o Serves as an advocate for the rights of the
client when barriers prevent obtaining needed care or the client is
unable to do so. o Supports pediatric triage, referrals, and
patient and family education as a Clinic RN. Health Coach-Primary
responsibility is to educate and maintain communication with
patients assigned to their caseload to facilitate self-management
of chronic and complex disease and improvement/maintenance of
quality of life. Medication Assisted Treatment (MAT) nurse case
manager-Provides skilled nursing services to opioid dependent
patients and their significant support systems. Provides case
management services for opioid dependent patients and their
significant support systems, including assessment, planning,
coordination, and follow-up. o Coordinates/assists with grant
specific or other required MAT documentation. o Provides education
and assessment to patients looking to access MAT treatment
programs. o Follows state and federal guidelines in providing care
to opioid dependent patients in collaboration with licensed,
prescribing physicians and pharmacy. o Assesses and monitors
patients in the induction, stabilization, and maintenance phases of
treatment. o Works with pharmacy and the MAT team in obtaining
medication orders, prescriptions, refills, prior authorizations,
and communication as needed. o Assists in the ongoing primary care
of patients referred to the opioid treatment program. o Facilitates
communication between all members of the care team, patient, and
family. o Manages patient referrals and follow-up to other
specialty medical, mental health, substance abuse and community
service. o Provides education and insight to staff and community as
needed related to substance use disorder and MAT options. o
Participates in local and national training and education to foster
continued professional growth and assure high quality care
delivery. • Supports a service-oriented atmosphere in accordance
with Peoples Community Health Clinic Mission and Philosophy. o
Demonstrates a cooperative and courteous manner towards patients,
families, and visitors while carrying out job functions. o Develops
an understanding of UDS and meaningful use clinical indicators and
Quality Assurance process while assisting care team members in
solving practice-related problems. • Works to improve work
processes and clinical outcomes including health disparity and
quality improvement collaboratives. • Follows policies and
procedures. • Develops and maintains own competence. • Maintains a
safe working environment and practices safe working habits. •
Assists in control of Peoples Community Health Clinic resources.
Competencies: • Strong analytical, problem-solving, and reasoning
skills and ability • Communication – listening, speaking, writing,
and reading comprehension • Attention to detail • Working knowledge
of MS Office Products (ex. Excel, Word) and Windows Platform. Must
be comfortable moving within an EHR computer system regularly •

Supervisory Responsibilities: • None Patient Population: Pediatric,
adolescent, adult, and geriatric patients. Work
Environment/Personal Protective Equipment: This position operates
in a clinical setting, might be exposed to bloodborne pathogen
hazard to head, hands, and torso. PPE is required. Physical
Demands: Alternately Sit/Stand–Frequently (34% - 66% of shift)
Walking-occasionally (1% - 33% of shift) Lifting 1 lb. to 75 lbs. –
Occasionally (1% - 33% of shift); Carrying 1 lb. to 20 lbs.-
Occasionally (1% - 33% of shift), Carrying 21 lbs. to 100 lbs. less
than 1% of shift. Push/Pull 1 lb. to 100 lbs. - Occasionally (1% -
33% of shift) Continuous fine motor skills including finger
dexterity and repetitive use of hands and arms. Requires 20/20 near
vision, 20/40 far vision, color discrimination and accurate depth
perception. Position Type: This is an hourly position covered under
the bargaining unit. Education and Experience: • Graduate of an
accredited nursing program. • One year of nursing experience is
preferred. Licenses and Certificates: • Current Iowa RN licensure •
Current BLS or CPR/First Aid certification • Mandatory reporter
training for child and dependent adult abuse certificate Other
