Position Type: Regular Scheduled Hours per 2 week Pay Period: 80 Primary Location: IA > DES MOINES > IHC LAUREL GENERAL SUMMARY: Coordinates and provides intermittent care for groups of high-risk individuals and/or populations in the inpatient and/or community setting. Assists other health care personnel in identifying health care needs. Interacts with patients and families and all levels of personnel, medical staff, community services, and insurance providers. May provide care to persons of diverse ages and cultures. Identifies and coordinates educational programs for patients and health care professionals, as well as the community. Interprets standards for the care of a specific patient population and provides clinical, consultative, educational and research support to ensure that quality cost-effective care is consistently provided. Communicates across health care areas to ensure that systems integrate efficiently in order to best serve patients, staff, physicians and community. ESSENTIAL FUNCTIONS: CLINICAL PRACTICE Acts as a liaison among the individual/family, community, provider and physician as well as other health care team members. CONSULTATION/EDUCATION Consults with interdisciplinary teams in the delivery of client/community care necessary to meet identified/negotiated goals across all health care settings. Collaborates in developing, implementing and evaluating teaching/learning strategies for client/family in the hospital community. Provides clinical leadership by being a preceptor for visiting professionals, students and new graduates. Coordinates appropriate educational material/instructions for patients, families, medical staff and multidisciplinary personnel to accomplish patient care outcomes. Establishes and coordinates patient/community rounds, as appropriate. Identifies specific needs for continuing education based on outcomes. Participates in planning and presentation of continuing education programs, seminars and meetings. MANAGEMENT/LEADERSHIP LEADERSHIP Utilizes expert knowledge and teamwork to facilitate cohesive efforts to achieve optimal outcomes in providing for patient/populations, and/or communities. EVALUATION Assists with the development, implementation and evaluation of clinical tools that targets time frames for expected outcome events determined through collaboration with health care team members. Coordinates with the health care team to develop a mechanism to monitor patient outcomes and follows up on variances. Ensures collection and input of risk stratification data. Reports results to the health care team and participates in interpretation of data and formulation of action plan. COORDINATION Collaborates with patients/families/populations to coordinate community services and appropriate resources which meet the individual needs and goals. Acts as a steward of resources in providing prudent health care services. PROFESSIONAL GROWTH PROFESSIONALISM Enhances knowledge and expertise and serves as a role model by affiliating with appropriate technical and professional organizations; by participating in applicable conferences, seminars and workshops; and through review of relevant literature and self-study course. Serves on selected hospital, group practice and community-based committees. Writes article(s) to be published in Clinician Newsletters and/or other print media. RESEARCH Identifies need for and participates in research projects relevant to specific patient population. INTERPERSONAL SKILLS COMMUNICATION Demonstrates exceptional communication skills and knowledge of conflict resolution. Supports the sharing of relevant resources. Utilizes information systems. Collaborates with Quality Management, Utilization Review, Information Technology Services, and other departments regarding data availability and reports. QUALITY MANAGEMENT/STANDARDS Is accountable for the implementation and communication of standards for patient care delivery appropriate to the specific patient population that meets professional and regulatory standards. Facilitates the monitoring of the quality of care by physicians, nurses and other members of the health care team. Coordinates the health care team in development of clinical tools for optimal outcomes and reports findings. Documents program specific outcomes, along with periodic revisions. CONFIDENTIALITY Maintains confidentiality of patient, personnel and institutional information.
