Position Type: Regular Scheduled Hours per 2 week Pay Period: 40 Primary Location: IA > DES MOINES > HOUSE OF MERCY GENERAL SUMMARY : Providesthe services to adults including: conducting face-to-face assessments,establishing diagnoses, making treatment suggestions; ordering behavior healthintervention services (when indicated); and assisting with referrals tobehavioral health intervention services providers (if requested); Provides substanceabuse, co-occurring trauma, and personal growth-related therapy services toassigned clients and their families. Applies advanced psychosocial theory andmethods to individuals as well as families and groups with appropriate regardfor the complexity of given problems. Performs tasks and makes decisions thatrequire independent judgment, are complex in nature, involve short and longrange planning and require the use of high level problem solving techniques.Applies concepts of client- centered care to improve client outcomes.Coordinates internal/external services to provide as complete a continuum ofcare as possible. ESSENTIAL FUNCTIONS : Establishes rapport anddevelops a therapeutic relationship with assigned clients. Provides therapy andcounseling services in individual and group sessions. Maintains appropriateprofessional boundaries. ·     Provides crisis intervention and conflictresolution assistance. Demonstrates ability to differentiate between emergencyand non-emergency tasks and responds accordingly. Provides immediate supportivecounseling and necessary assistance to clients dealing with a crisis oremergency situation. Assists with referrals to community resources whenappropriate. Demonstrates the ability to perform in an accurate and precisemanner in situations where time is critical. ·     Conducts educational groups providingrecovery-related information and instruction. Assists with referrals to community resourceswhen appropriate. Provides counseling/treatmentservices to restore clients to optimum social and physical well-being (withintheir capacity). Provides counseling in individual and groupsessions for co-occurring, trauma and personal growth related concerns. Provides family, couples,and/or relationship counseling services in designated situations. Completes an initial assessment that includes evaluation of theAmerican Society of Addiction Medicine Patient Placement Criteria (ASAM).Obtains bio-psychosocial history information and identifies client needs byinterviewing the client, reviewing available records, conferring with otherprofessionals and appropriate House of Mercy staff, and contacting otherinformation sources as necessary and appropriate. Analysis and evaluation ofclient situation consistently reflects advanced competency in psychosocialassessment and diagnosis. Assesses consistently reflects advance competency inpsychosocial assessment and diagnosis. Assesses problems appropriately in termsof scope, consequences and needed interventions. Performs a comprehensive assessment(psychosocial history) to analyze and synthesize the client’s status andaddresses the client’s strengths, problems and areas of clinical concern. On anongoing basis, assesses client needs/strengths, behaviors, and progress towarddesired outcomes. Specifically monitors client progress in areas of personalgrowth, education, prenatal care, parenting, employment, living skills, etc.Determines need for outside professional evaluations (e.g. mental health), andindependently refers to community practitioners/agencies as needed. Develops treatment plans based upon assessment and in accordance withprogram requirements, state regulations, etc. Communicates pertinent treatmentplan information to clinical staff and others as needed. Implements plan andmonitors patient’s progress. Participates in case review meetings to review and discuss client progress,assure consistent implementation of treatment plan goals/objectives, andpromote team involvement in problem solving/resolution. ·     Coordinates services with communityagencies, schools, courts, service providers of significant others, and mentalhealth/other health care professional to provide an extended, high qualitycontinuum of care for House of Mercy adult residents. Consults with House of Mercy staff and client to determine client’spost-discharge needs and plans. Coordinates services with community agencies asneeded to provide an extended, high quality continuum of care, a broad spectrumof discharge planning services and collaborates with treatment team inimplementing appropriate discharge plans and aftercare programming. Providessupport, encouragement as needed throughout the process. Maintains an awareness of applicable laws, regulations and licensingrequirements as they relate to resident documentation. Assumes accountabilityfor providing appropriate documentation of all therapy/counselingsessions, significant incident occurrences, phone conversations, etc.consistent with applicable regulatory requirements. ·     Prepares and distributes required reports and other documentationof services according to House of Mercy policies and procedures, applicablelaws, regulations and licensing requirements. Responsible for the maintenance ofclient files. Assures all needed releases of information and other paperworkare present in the file and up-to-date. Promptly requests new or additionalinformation and paperwork as needed. ·     Assures that Resident Counselors, Adult ProgramsCounseling Services and Internship/Staff education Supervisor, Director andother House of Mercy staff (as appropriate) have the information they need toprovide quality service to the client; assists in the training of ResidentCounselors. MINIMUM KNOWLEDGE, SKILLS AND ABILITIESREQUIRED : Bachelor’s degree in a human services related discipline from anaccredited four-year program, required. ValidDriver’s/Chauffeurs License required, must meet Mercy’s Motor Vehicle SafetyStandards, must be at least 18 years of age and be eligible to drive per Iowastate law. Proof of completion of Mandatory Reporterabuse training specific to population served within thirty (30) days of hire. Licensed to provide mental health therapy inthe State of Iowa (LMHC, LISW, LMFT, etc.) and meets the requirements necessaryfor an LPHA designation OR CADC certification within one (1) year of hire.
